Intermittent/Random noise emitted from outputs

I recently had a Tsunami Wav trigger installed in a gallery. The device is playing back three looped audio files. The trigger points are connected so when the device is powered, the playback is triggered instantly. It was working fine for a few weeks, and I got word from a curator that random loud noise is emitted (sounding a lot like feedback, but they couldn't give me much more information than that). It is apparently so disruptive that they have had to leave the device off.

In a former job i dealt with sound issues a lot..... this could be nearly anything....

it could be that the wire that is connected between this box and the amplifier is not shielded enough, picking up noise on it's way,
it could be the power supply not strong enough causing uncontrolled logic of a "huuuummmmm", (on a 50/60 hz frequency)
it could be that noise is entering the box from an unknown source nearby (it looks to be a plastic box, instead of shielded)

What is mend with "sounding a lot like feedback" ? How does that sound ? Does this happens continuous or on certain moments ?

If indeed it "was working fine for a few weeks" and then started doing this, this implies that something has changed or broken. How are you powering the Tsunami? Does that box get closed up with no ventilation? Is the noise on all the outputs or just one? Does cycling the power always fix it?

Also, what makes you sure it's the Tsunami board? What about the amplification and speaker system downstream from the Tsunami's audio output?
